Our team
Our multidisciplinary team brings together epidemiologists, health experts, communications leaders, patient and clinician engagement specialists, event strategists, grant and development professionals, and thought leaders. Together, we combine research, lived experience, clinical insight, storytelling, partnership development, and community engagement to design impactful health equity programs, strengthen trusted relationships, and improve health outcomes.
Our mission
Rooted Health Rx is a community-centered nonprofit advancing health equity through culturally responsive education, screenings, care navigation, and wellness programs. We partner with health systems, academia, government agencies, nonprofits, community organizations, and brands to connect individuals and families with trusted healthcare providers, resources, and support services that help communities live healthier, more empowered lives.
