The Health Fair and Listening Labs Program combines culturally responsive health education, patient screenings, and community-centered conversations to better understand local health needs. Through screenings and facilitated listening labs, the program gathers patient experiences, identifies barriers to care, and connects participants to relevant healthcare providers, community resources, and social support services. Rooted Health Rx also delivers actionable insights, comprehensive reporting, and culturally relevant educational materials to help partners strengthen outreach, improve services, and advance health equity.

The Patient + Clinician Education Program delivers culturally responsive workshops, trainings, and educational resources across multiple disease areas and health conditions. Patient education supports health literacy, prevention, screening awareness, treatment knowledge, informed decision-making, and stronger communication with providers. Clinician-focused programming includes workshops, trainings, and continuing education opportunities designed to strengthen cultural responsiveness, improve patient engagement, address barriers to care, and advance more equitable, patient-centered healthcare delivery.

Rooted Health Rx’s Wellness + Health Hub is a culturally centered community experience that connects individuals and families to trusted health education, preventive screenings, wellness activities, healthcare services, and local resources. Designed in collaboration with community leaders, clinicians, brands, and corporate partners, the Hub creates an engaging and accessible environment where participants can learn, connect, and take meaningful action toward better health.

Each activation can be customized around priority health areas and may include screenings, interactive workshops, fitness and wellness experiences, product education, patient navigation, culturally relevant resources, and connections to clinical and social support services. The Wellness + Health Hub provides partners with an authentic opportunity to build trust, deepen community engagement, increase brand visibility, and demonstrate a measurable commitment to health equity and community well-being.

The Community Health Worker Patient Navigation Program helps patients overcome barriers to care and successfully access the health and social services they need. Trained community health workers provide culturally responsive, one-on-one support, including health education, appointment scheduling, referral coordination, insurance and benefits assistance, transportation support, and connections to trusted clinical and community resources.

Through ongoing follow-up and warm handoffs, community health workers help patients understand their care plans, complete recommended screenings, access treatment, and remain engaged throughout their healthcare journey. The program also tracks patient needs, referrals, service utilization, and outcomes to provide partners with actionable insights and reporting that support improved care coordination and health equity.

The Health Campaign Program develops culturally relevant, community-centered campaigns across multiple health issues and disease areas. Through trusted messengers, digital and social media content, educational materials, community activations, and strategic partnerships, each campaign raises awareness, promotes prevention and early detection, and connects individuals to screenings, healthcare providers, patient navigation, and local support services.

Campaigns can be customized to align with a partner’s priority populations, health focus areas, and geographic markets. Rooted Health Rx also provides engagement tracking, community feedback, insights, and outcome reporting to help partners measure reach, strengthen health education, and advance meaningful health equity goals.

Rooted Health Rx’s Advocacy Program elevates patient and community voices to advance policies that improve healthcare access, quality, and equity across multiple health and disease areas. The program brings together patients, caregivers, clinicians, community organizations, industry partners, and policymakers through advocacy trainings, listening sessions, policy briefings, roundtables, and storytelling initiatives.

Through congressional engagement—including meetings with elected officials and staff, Capitol Hill briefings, advocacy days, and policy-focused events—participants share lived experiences, highlight barriers to care, and advocate for community-informed solutions. Rooted Health Rx also develops advocacy resources, policy insights, engagement strategies, and impact reporting to help partners strengthen awareness, build coalitions, and support meaningful policy change.

The Creative Health Connection Initiative brings together the health, arts, cultural, and creative sectors to make health education, resources, and community engagement more accessible, culturally relevant, and meaningful.

The initiative uses music, visual arts, storytelling, performance, cultural experiences, and creative expression as tools to engage communities in conversations about health and wellness, increase awareness of healthcare resources, strengthen connections to care, and support overall quality of life.

Rather than focusing on a single disease or population, the initiative addresses health across the lifespan, with disease- and population-specific programs operating as subsets of the larger platform.
